IMPORTANT: THERE ARE TWO (2) STEPS NEEDED TO BECOME CLEARED!

THESE MUST BE DONE EACH ACADEMIC YEAR!
STEP 1 OF THE ATHLETIC CLEARANCE PROCESS:
Access and complete your "FamilyID Registration" by clicking HERE!
You enter your information only once for each family member for multiple uses and multiple programs. There is no need to complete a new registration for each sport season; you may simply UPDATE through the school year as information changes!
At any time, you may login at www.familyid.com to update your information and to check your registration(s).
To view a completed registration, select the 'Registration' tab on the blue bar.
STEP 2 OF THE ATHLETIC CLEARANCE PROCESS (COMPLETED ONCE PER SCHOOL YEAR):
ATHLETES MUST PICK UP A HARD-COPY OF THE CIF PHYSICAL FORM "GREEN CARD" FROM THE ATHLETIC OFFICE. AFTER THE PARENT/GUARDIAN COMPLETES AND SIGNS THE ATHLETE'S MEDICAL HISTORY ON THE FRONT, THE DOCTOR WILL COMPLETE THE PHYSICAL ON THE BACK. THE PHYSICAL MUST BE SIGNED BY AN MD OR DO AND DATED ON OR AFTER MAY 30, 2022. THE PHYSICAL FORM MUST BE HANDED TO THE ATHLETIC DIRECTOR OR SHEILA FOR REVIEW AND APPROVAL. DO NOT TURN THIS IN TO ANYONE ELSE OR LEAVE IT IN A MAILBOX.
**Faxed or emailed physical forms are not accepted to ensure the authenticity of the information and signatures. Only a physical copy on the Green Card is accepted!
The GREEN Card is a legal document and must be completed in blue or black ink.
