When is my athlete's athletic packet due?

Athletic clearance and physical forms are due to the athletic director before your athlete participates in any tryouts, workouts, conditioning, practices, and games. Coaches have access to athlete-clearance lists and will not allow uncleared athlete to participate.

Who accepts them?

The athletic director, Lindsay Franklin, and ASB/Athletics Clerk, Sheila DeLisle, are the ONLY people who accept the physical examination forms. They must be submitted in person (CANNOT be left in the office or under the door). Coaches and other staff are instructed NOT to accept packets.

For more information, check out the "ATHLETIC CLEARANCE" page, FAQ page, or continue visiting this page!

Title IX Enrollment Data

revised 6/20/18

THANK YOU for considering to become a volunteer driver for TUSD. Fingerprinting can take anywhere from 1-3 weeks to clear, so don't wait! Here are the necessary steps:

1) Bring a copy of your driver's license, insurance, and H-6 (driving record printed at the DMV) to the ATHLETIC DIRECTOR. The printed driving record you can obtain online cannot be accepted.

2) You will be asked to sign a DMV Pull Notice consent form, enabling TUSD to pull your driving record in the future, saving you future trips to the DMV!  You will also sign the Van Driver Contract and submit to the athletic director.

3) Upon approval (no more than 2 points, no DUIs or reckless driving, at least 21 years old, etc.), you will be asked to go to district office to complete TB test and obtain DOJ Fingerprinting voucher (both in the VOLUNTEER PACKET you will receive). If you have done fingerprints with the Office of Education in the past, they still may be accessible, so don't forget to ask!

4) Once your volunteer packet has been completed and submitted to the district office, and fingerprints/TB test clear through the Dept. of Justice, the advisor of your group will be notified of your clearance.

Once again, thank you for becoming a volunteer driver. Additional questions can be directed to

ATHLETES: Please review the athletic attendance policy--you must attend 2 periods on practice days and you MUST attend all periods on the day of a contest until you are released. Doctor or dentist visits with a signed note by the doctor are the ONLY valid excuses for missing class on game day. Personal reasons or illness (even with a parent note) are NOT excuses and you will be ineligible for your next contest. If you have questions, please see Coach Franklin.
